This service is available to people living in Coventry and Rugby who are providing care and support for a person with a diagnosis or likely diagnosis of dementia.

To access the service, patients or carers must be registered with a Coventry or Rugby GP.

Referrals will be accepted from Patients, Carers, GP’s and Arden Memory Services.

Six Admiral Nurses serve the two areas thanks to a partnership project between NHS Coventry and Rugby Clinical Commissioning Group (CCG), Dementia UK, Coventry and Rugby GP Alliance and Coventry University.

Admiral Nurses are registered specialist dementia nurses who provide expert practical, clinical and emotional support to families living with dementia. They play a unique role in care management by joining up the different parts of the health and social care system, so the needs of family carers and people with dementia can be addressed in a co-ordinated way.

Please see right for the Admiral Nurse referral form and an information leaflet about the service.
