Urgent Phlebotomy Appointments for GP Practice Teams
There are a number of phlebotomy appointments allocated for practices to book for patients who require an urgent blood test on the same day, or the next day
An urgent blood test is appropriate if a test result within 24hrs is needed to inform a crucial clinical decision about a patient’s immediate care, for example, the test will potentially help to avoid an admission to hospital

Patients Booking an Appointment for Blood Tests
Phlebotomy clinics are now operating on an appointments only basis
Walk in patients who attend clinics will be asked to make an appointment for their blood test
Patients can book at www.uhcw.nhs.uk/bloodtests – for all clinics across Coventry and Warwickshire
The map shows all our locations across Coventry and Warwickshire where you can also book a blood test appointment.

Family members and carers can also book through the online system on behalf of their relatives and dependents
For the minority of people who do not have internet access to book online, there is a telephone booking line – 024 7615 3546 – please note the line has limited capacity so is only for those without internet
Warwick Hospital is requesting that patients with GP blood test requests do not come onto the Warwick Hospital site solely for a blood test, please direct patients to Stratford or RSX clinics
Practices and Clinics can directly book patients into blood test clinics, if they so wish – it is quick and simple

Booking of appointments at community facilities
Online booking is available at various community pharmacies and community facilities in addition to UHCW outpatient phlebotomy services
University Hospital, Coventry
Patients are advised to use the new online appointment system before attending for a blood test
The new service helps to reduce clinic waiting times and enabling patients to be seen much more quickly.

Opening hours: 8am-4.45pm, Monday to Friday (excluding bank holidays)
Coventry Guidance
City of Coventry NHS Walk-in and Healthcare Centre Phlebotomy Service

Location: Access is via the main doors, the Phlebotomy team are in Area A
Opening hours: Monday to Friday (excluding bank holidays). 8am–4.45pm
Children under 16 years old will need to book online here.
Domiciliary Phlebotomy Service Referrals
The service will support all HOUSEBOUND patients [those unable to access alternative phlebotomy services] if registered with a Coventry GP

PLEASE NOTE: Telephone referrals will no longer be accepted, any queries, please contact service for further advice 02476 961380
All URGENT requests for same day (or within 24 hours) should be reserved for admission avoidance cases ONLY and a referral form should be emailed to: dombloods@covwarkpt.nhs.uk followed up with a phone call to 02576 961380 to ensure allocation of request
Any urgent referrals received Mon-Fri after 13:00 will automatically be allocated to the next working day

Rugby & South Warwickshire Guidance
The Friends Blood Taking Unit, Hospital of St Cross
Patients from Rugby and the surrounding areas are able to access the Friends Blood Taking Unit at the Hospital of St Cross
Referrals for blood tests can be from GP’s, practice nurses or from hospital consultants. Patients are also able to leave other samples, such as urine, which have been collected at home, at the hospital’s Pathology Reception

Monday – Friday 8am-4.45pm Appointments only
Wednesday Evening 5pm-7pm Appointments only

Please bring a current blood test request form with you. If you do not have one, please contact your hospital consultant or GP and they will post one to you. Unfortunately, your blood test will need to be rescheduled if you do not have a blood request form when you come to a Blood Test clinic
