Community management advice for difficult diabetic and lipid cases


 

The Community Diabetes and Cardiovascular Risk Clinic Service

Spires Suite at City of Coventry Health Centre, 2 Stoney Stanton Road, Coventry CV1 4FS.

Referral via the NHS eReferral Service (clinic is called “Community”) for patients with a Coventry registered GP.

Service provided by General Practitioners with Specialist Interest (GPwSI):

Dr Jim McMorran and Dr Mukesh Mistry

Difficult to manage diabetes patients including:

  • Suboptimal glycaemic control despite three oral hypoglycaemic agents
  • Initiation of newer therapies such as SGLT2 inhibitors, GLP1s
  • Consideration for conversion to injectable therapies
  • Difficult to manage lipid problems in diabetic patients
  • Patients with unstable glycaemic control

Difficult to manage lipid problems including:

  • Statin intolerance
  • Mixed hyperlipidaemia
  • Hypertriglyceridaemia
  • Possible familial dyslipidaemias such as familial hypercholesterolaemia and familial combined hyperlipidaemia
  • Assessment of cardiovascular risk with specialist markers such as lipoprotein a, apolipoprotein B levels
  • Management of low HDL
  • Initiation of combination lipid lowering therapies such as combining a statin and fibrate
  • Management of combined dyslipidaemia in renal impairment
