Please note- EMIS form Only:

1. The referral form cannot be opened outside of EMIS, please SAVE UNOPENED DOCUMENT on your desktop or in downloads then import the form using EMIS TEMPLATE MANAGER and then you can view the form within your clinical system
2. When you import the form, please ensure when saving the form, you click ‘YES’ to protecting the form. This will appear as a prompt within EMIS. This will ensure any tick boxes work correctly.
