About the service

Urgent Community Response in Warwickshire aims to prevent unnecessary hospital admission for patients who are frail and / or have chronic long term health conditions

Fallers on anticoagulants

Since 1st December 2025a new clinical pathway has been added to the UCR service to support patients who have had a fall and are taking anticoagulants

Supported by the Frailty Teams at Warwick and GEH, more patients are now able to be treated at home, avoiding unnecessary hospital conveyance

The new pathway is available countywide for new referrals 8am – 6pm, 7 days a week as part of normal UCR delivery
