Implementation of the Patient Safety Incident Response Framework (PSIRF) shifts from reporting of incidents via DATIX to reporting via a new national platform, called Learning from Patient Safety Events (LFPSE) system

However, whilst the PSIRF is a contractual requirement under the NHS Standard Contract, it is not a contractual requirement within primary care at this stage

The Learning from Patient Safety Events (LFPSE) service is a new national NHS service for the recording and analysis of patient safety events that occur in healthcare. It has replaced the National Reporting and Learning System (NRLS). It is hoped that the new system will increase reporting and allow for more effective learning from patient safety events

In order to implement the change from DATIX to LFPSE the ICB will be decommissioning DATIX

However, Primary care staff are now able to utilise the LFPSE system to record any events where:

  • a patient was harmed or could have been harmed.
  • there has been a poor outcome, but it is not yet clear whether an incident contributed or not
  • risks to patient safety in the future have been identified
  • safe and effective care has been delivered that could be learned from to improve patient safety

The ICB will transition from Datix to LFPSE system in January 2024, therefore from January 31st, 2024, Coventry and Warwickshire GP practices can start to migrate across to the new LFPSE system if they wish

To do this Practices can register /create an online account with the LFPSE system via Learn from patient safety events (learn-from-patient-safety-events.nhs.uk)
