Community AF Service in Coventry and Warwickshire
A new fast track service for Atrial Fibrillation (AF) has been set up at Hartshill Health Centre (HHC)
The aim is to prevent strokes by speedy initiation and titration of medication for patients with AF
Please refer:
- All new patients diagnosed with AF – ECG is required for confirmation of diagnosis (AF treatment should be commenced where possible as per national/local guidelines)
- Any patient with established AF who has not had an Echocardiogram
Your patients will be seen within 2 – 4 weeks for treatment and Echocardiogram with subsequent onward referral to secondary care for interventions such as cardio-version or left atrial appendage closure procedure if required
This service is run by specialist AF nurses in collaboration with Dr Sandeep Panikker, Consultant Electrophysiologist Cardiologist at UHCW and Lead for AF across Coventry and Warwickshire
Although some patients may need to travel longer distances to attend the clinic, the benefits to the patient will be:
- Rapid access and better AF/stroke outcomes
- Detailed review as per NICE guidance (45 mins)
- One stop investigations (ECG, echo etc)
- Specialist arrhythmia review
- Holistic AF risk factor management
- Review via F2F or telephone F/U appts
- Dedicated Community AF Service phone line for patient support and queries once reviewed
