Incident Reporting
Responding appropriately when things go wrong in healthcare is a key part of the way the NHS can continuously improve the safety of patients.
CWICB have commissioned DATIX system to report incidents, this enables all practice staff to report incidents that occur within the practice and other providers. As an ICB we monitor themes and trends at a system level collaborating with other quality leads to triangulate information and cascade lessons learnt or examples that have improved patient safety.
We will begin to share with you the reporting charts from Primary Care along with relevant case studies on a quarterly basis
Below you will find the main Themes reported through Datix this past year (2021/22) and the actions that were taken by the Quality Team.
Reporting a Clinical Incident
If a practice encounters any patient safety incidents, they must report this via the practice’s usual incident reporting (e.g. Datix, LFPSE).
For South Warwickshire practice incidents should be reported via: cwicb.qualitymatters@nhs.net
